What do I need to bring on my first appointment?

All medication bottles. Insurance card, ID, and be prepared to provide a urine sample.

Do I have to pay my copay upfront?

Yes.

What if I can't pay my bill at time of service?

You have the option to add a $10 service charge.

Can I choose which doctor I see?

Yes.

Will I only see a doctor during my appointments?

Yes, unless scheduled w/ an Advanced Nurse Practitioner.

What if I miss my appointment?

We will kindly reschedule your appointment at no charge.
